If memory serves, it showed up first on the Walk Away 12" and - I think - was used on the Black October tour. Given the tendency to - ahem - appropriate cover art from various sources, I wouldn't be surprised if this had originally come from a print or painting or similar.

as a professional designer (they pay me for this shit) I have searched long and hard for reprazent!ations of the Black October symbol and only two ring true, and they're missing the bar

I found this symbol before many years in a book of astronomy.
According that book, this is the symbol of Charon planet, which is a kind of Pluto's moon.
Charon was also the ferryman who ferried the dead to the underworld (in Greek mythology).
I took a quick look on the net and I didn't find the symbol exactly.
But I found that:
