The mess that scalpels make is none other than laughing Lenny himself. In previous verses he speaks about using his fork like a scalpel in an asylum and asking God if he’s carved enough. Eldritch is right, he’s a regular Bill Hicks.

Came here because I've been listening to this song all day. I write and I borrowed SGWBM as a title and I was curious what was said here.

There is no destination. Carve and fatten and try. Some wander into this journey.
