I'm getting feedback that the Capcha is overly difficult to get right. I just tried to re-register with a spare email address and it took me three goes...

You can configure it within the admin section...
All the added effects (colours, backgrounds, rotating letters, etc) are there to make it harder for bots to decipher, but tbh most of the added effects have already been disabled
I've turned off the last optional effect setting.
This is what it looks like now:
Not sure what else I can do tbh, other than just turning CAPTCHA off.
CAPTCHAs are known to be awful for usability, but they do tend to work enough of the time to warrant keeping them - see how many spammers we get these days compared with how it used to be?
if it's still an issue, we could try dialling it for a week and see how many spammers we get... but you'd need to be constantly checking & vetting new signups...

20 new 'users' in roughly 17 hrs, and 14 of them have 'activated' their accounts and could (in theory) post...We have 3920 registered users

Fwiw, is it impractical to have a registration queue?
We have one set up on TCC and it does help.
Only PITA is needing to check it and manually verify new members.
Also useful to filter out duplicate registrations.
it also uses the stop forum spam site to do a check on ips and email addresses.
